What Is Intensive Case Management?

At C-U at Home, housing is just the beginning. Real transformation requires guidance, encouragement, and consistency. That’s why intensive case management is one of the cornerstones of our Pathways to Progress program.


But what does that actually mean?


Not Just Paperwork: It’s a Relationship

Case management isn’t just about filling out forms or keeping tabs. It’s about building trust. Every client at C-U at Home is paired with a case manager who becomes their teammate, advocate, and encourager throughout their journey.


Here’s what intensive case management looks like in action:


  • Three structured meetings per week in Phases 1–3

  • Weekly goal check-ins and progress tracking

  • Assistance navigating health care, legal issues, education, and employment

  • Emotional support through life transitions

  • Accountability when it gets tough—and celebration when it gets good

We help clients define their own success, then walk alongside them to reach it. For some, it means reconnecting with their children. For others, it’s staying sober for 30 days. For many, it’s landing a job or finally getting an ID after years without one.

We don't take over—we equip, coach, and cheer.
