Case management isn’t just about filling out forms or keeping tabs. It’s about building trust. Every client at C-U at Home is paired with a case manager who becomes their teammate, advocate, and encourager throughout their journey.


Here’s what intensive case management looks like in action:


  • Three structured meetings per week in Phases 1–3

  • Weekly goal check-ins and progress tracking

  • Assistance navigating health care, legal issues, education, and employment

  • Emotional support through life transitions

  • Accountability when it gets tough—and celebration when it gets good
