Intensive Case Management
To participate in the Mid-Barrier program, a client must agree to participate in an intensive case management plan for all four phases of the program. Upon entry, each client is assigned a case manager. The case manager will be assigned to the client for the duration of the client’s stay. The case manager assigned to each client has experience working with clients who have substance abuse, mental health, and trauma. The case manager meets with a client to begin a client-centered, trauma-informed case plan. Depending on the client’s needs the goals and interventions will address issues regarding life skills, health and legal issues, identification, shelter responsibilities, future housing plans, etc.
During phases one to three, clients will meet with their case manager at least 3 times per week. During phase four, meetings will be adjusted based on a client’s need. These meetings are to assist the client in implementing interventions to meet their goals and adjusting goals and interventions as needed. Throughout this time, the case manager and client will assess the client’s progress in the case plan. The case manager and client will collaboratively adjust the case plan as needed.
