Advanced Program
The final phase of the program provides for participants to live independently in one of the C-U at Home’s five homes within the community. In this phase of the program the clients meet with their case manager weekly at first with a gradual decreased need for case management. The case manager would develop goals and work with the client until permanent housing is found. At this phase, clients are asked to begin contributing to household costs by paying a portion of their income. Clients are allowed to remain in this phase for up to 12 months.
Clients have access to the Independent Living Houses 24/7, 365 days/year. Clients may be asked to leave if they violate safety procedures, use substances on the property or refuse to work within the case management system.
